This individual will serve as the first point of contact for patients and caregivers, helping explain the program, guide them through enrollment, and ensure a positive patient experience throughout every interaction. • Contact patients and caregivers to explain care management programs and guide them through the enrollment process
• Communicate program benefits in a warm, professional, and patient-friendly manner
• Follow outreach and scheduling workflows with accuracy and attention to detail
• Maintain patient engagement metrics including consent rates, scheduling rates, and no-show recovery
• Collaborate with internal teams to improve workflows and patient experience initiatives
• Provide feedback on outreach and scheduling technology tools
• Maintain professionalism, empathy, and strong communication during all interactions
• Support a patient-first culture focused on delivering high-quality care experiences
